Housing and Shelter . . . Top

Housing Choice Voucher (Formerly Section 8) - Provides decent, safe, and affordable housing to low-income residents.  The program is designed to provide vouchers allowing families to locate a unit of their choice that falls within Housing Quality Standards rather than in public housing project.  The monthly subsidy amount is a portion of the rent, based on the family’s income and is paid directly to the landlord.   Families that fall within the 50th percentile of the median area income are eligible.  Preferences are given to elderly, disabled, and disaster victim households. Funded by U.S. Department of Housing and Urban Development.

Supportive Housing Program - Designed to stabilize housing, increase income and skills, and improve the self-determination of homeless persons and families through rental assistance and case management services.  The program targets homeless individuals with a disabling condition including physical, mental or emotional impairment, a developmental disability, and a diagnosable substance abuse disorder.  Homeless persons in Bledsoe, Bradley, Grundy, Marion, Meigs, McMinn, Rhea, Polk, Sequatchie, and Hamilton County are eligible to participate in the program. Funded by U.S. Department of Housing and Urban Development.

Shelter Plus Care- Provides decent, safe, and affordable housing to homeless individuals and families.  This program targets individuals and families who are homeless and who have serious mental illness and/or chronic problems with drug and alcohol.  Benefits are available to qualifying residents in Bledsoe, Bradley, Grundy, Marion, Meigs, McMinn, Rhea, Polk, and Sequatchie Counties. Funded by U.S. Department of Housing and Urban Development.

Cedar Grove Way Apartments - One bedroom rental housing in the City of Dunlap to help participants that are diagnosed with mental illness and/or recovering from addition or alcoholism and are homeless.  Preference is given to participants of the Adult Community Corrections and Misdemeanor Probations programs.  Available in Sequatchie County. Funded by Tennessee Housing Development Agency.

Low-Income Home Energy Assistance – This program assists low income families/individuals, elderly, or handicapped persons with energy needs.  Assistance may be provided in electric, natural gas, fuel oil, kerosene, wood and coal.  Payment is made directly to energy or fuel supplier.  Low income residents in Bledsoe, Grundy, Marion, Meigs, McMinn, Rhea, Polk, and Sequatchie counties may apply for services. Homeless Assistance- The program provides services for individuals and families who are without a fixed residence.  Clients are assisted with first month’s rent and with utility deposits if they do not have means to help themselves.  Low income residents in Bledsoe, Grundy, Marion, Meigs, McMinn, Rhea, Polk, and Sequatchie counties may apply for services. Funded by the Tennessee Department of Human Services.

Crisis Intervention- The program assists low income individuals in an emergency situation by providing rent, utilities, mortgages and food.  Residents in an emergency situation in Bledsoe, Grundy, Marion, Meigs, McMinn, Rhea, Polk, and Sequatchie counties may apply for services. Funded by the Tennessee Department of Human Services.

Project Hometown- Assists customers of Middle Tennessee Natural Gas in Bledsoe, Grundy, Meigs, and Rhea Counties who have an appropriate disconnect/cutoff notice. Funded by and Middle Tennessee Natural Gas customers

Project Help – Assists customers of Sequatchie Valley Electric Co-op in Bledsoe, Grundy, Marion, and Sequatchie counties who have an appropriate disconnect/cutoff notice. Funded by and Sequatchie Valley Electric Co-op customers.

FEMA- Assists low income individuals and families with rent, utilities, mortgages, and food when in an emergency situation.  Applicants must provide utility cut off notice and/or eviction notice.  Applicants must reside in Bledsoe, Marion, Meigs, Polk and Sequatchie Counties.  Funded by Federal Emergency Management Agency.

Child and Adult Care Food Program- The program provides reimbursement for meals to child care providers who operate in-home day cares.  Day care providers must be licensed by the state or alternate approved by the agency to participate in the food program.  Day care providers throughout the state are eligible to participate in the program.  Funded by the U.S. Department of Agriculture and administered by the Tennessee Department of Human Services.

Nutrition Services – The program provides at least one hot or appropriate nutritionally-balance congregate or home-delivered meal a minimum of five days per week. Other nutrition services include nutrition screening, assessment, education and counseling, and nutrition education. Eligibility is available to older consumers or adults with disabilities who live alone in Marion and Grundy Counties. The Nutrition Services program is funded through Title III – C under the Older Americans Act.

Commodities Foods- Government surplus foods are distributed on a quarterly basis to low-income, no-income or disabled individuals to help increase their food supply and provide nutritious meals.  Individuals in Bledsoe, Grundy, Marion, Meigs, McMinn, Rhea, Polk, and Sequatchie counties may apply for services. Funded by the U.S. Department of Agriculture.

Family Self-Sufficiency- The program provides financial assistance for employment and education related expenses to low-income families.  Assistance helps families enter or maintain employment.  Services include, but are not limited to mileage reimbursement, child care, uniforms, and auto repair. Any household in Bledsoe, Sequatchie, Marion, Grundy, Meigs, McMinn, Rhea, and Polk counties that meet the income guidelines may apply for services. Funded by the Tennessee Department of Human Services.

Pharmaceutical- Provides financial assistance for purchase of prescription medications, nutritional supplements, and ambulatory devises to elderly households.  Any elderly household in Bledsoe, Sequatchie, Marion, Grundy, Meigs, McMinn, Rhea, and Polk counties that meet the income guidelines may apply for services. Funded by the Tennessee Department of Human Services.

Weatherization - The weatherization program provides a whole house approach that incorporates advanced technologies and addresses the comprehensive energy usage in low-income homes, as well as related health and safety improvements. In order to be eligible for this program, you must provide proof of income and be in need of home, and/or safety improvements to your dwelling and live in Bledsoe, Grundy, Marion, McMinn, Meigs, Polk, Rhea or Sequatchie County.  Funded by Tennessee Department of Human Services. Elderly Support Services . . . Top

Homemaker Services – Provides normal household chores to elderly and disabled households in the service area. Any elderly or disabled household in Bledsoe, Sequatchie, Marion, Grundy, Meigs, McMinn, Rhea, and Polk counties that meet the income guidelines may apply for services. Funded by the Tennessee Department of Human Services.

Family Care Giver Support- The program provides caregiver support services such as homemaker to adult family members, or other individuals who are informal providers of in-home and community care to older persons. Caregiver support services are also available to grandparents or older individuals who are relative caregivers for a child, age 18 and under. Services are authorized through referral from the Southeast Tennessee Area Agency on Aging and Disability. Funded by Title III – Part E under the Older Americans Act.

OPTIONS for Home and Community Based Services- The program assists frail older adults and adults with disabilities to remain in their own homes, avoiding premature institutionalization.  Supportive services may include homemaker services and personal care assistance.  Funded in part by the Older Americans Act and the Tennessee Commission on Aging and Disability through the Southeast Tennessee Area Agency on Aging.

ARRA Minor Home Repairs - Provision and installation of certain home mobility aides (e.g., ramps, rails, non-skid surfacing, grab bars, and other devices and minor home modifications which facilitate mobility and safety) and modifications to the home environment to enhance safety.  All services shall be provided in accordance with applicable State or local building codes. Funded by Tennessee Department of Human Services.  Services are available to elderly (62 or over) or disabled households in Bledsoe, Bradley, Grundy, Hamilton, Marion, Meigs, McMinn, Rhea, Sequatchie, and Polk counties. Medicaid Waiver – This Waiver provides a community-based, cost-effective alternative to institutional nursing facility care for individuals who are eligible for level one nursing home care and are financially eligible.  Funded by Bureau of TennCare.  Services available in Bledsoe, Bradley, Grundy, Hamilton, Marion, Meigs, McMinn, Rhea, Sequatchie, and Polk counties and include:

  • Homemaker Services (HMKR) - services consisting of general household activities and chores and errands essential to the Enrollee's care provided by a trained homemaker.
     

  • Personal Care Services (PCS) - services provided to assist with activities of daily living, and related  essential household tasks, and other activities that enable the Enrollee to remain in the home, as an alternative to Nursing Facility care, including the following:
     

  • Minor Home Modifications (MODS) - the provision and installation of certain home mobility aides (e.g., ramps, rails, non-skid surfacing, grab bars, and other devices and minor home modifications which facilitate mobility) and modifications to the home environment to enhance safety.  All services shall be provided in accordance with applicable State or local building codes.
     

  • Respite Care (IR) - Services provided in-home, to individuals unable to care for themselves, furnished on a short-term basis because of the absence or need for relief of those persons normally providing the care.
     

  • Personal Care Assistance/Attendant - Intermittent provision of direct assistance with the activities such as toileting, bathing, dressing, personal hygiene, eating, meal preparation (excluding the cost of food), budget management, attending appointments, and interpersonal and social skill building to enable the enrollee to live in a community setting.

Corrections . . . Top

Community Corrections - Assists in diverting felony offenders from the prison system.  Monitoring of court costs, fines, and victim restitution; random drug screening; public service work; placement in drug/alcohol treatment programs; house arrest; continuing education services; and values clarification treatments are all components of the program.  Referrals to this program are made through the court system for those who have committed felony offenses in Bledsoe, Grundy, Marion, Rhea, Sequatchie and Franklin Counties. Funded by the Tennessee Board of Probation and Parole and offender supervision fees.

Victim-Offender Reconciliation Program (VORP)- VORP services will be provided in the 12th Judicial District (Bledsoe, Franklin, Grundy, Marion, Rhea and Sequatchie Counties) .  Mediation is a private and voluntary process that allows individuals to resolve their own problems with the help of a trained, impartial third person.  Volunteer mediators will assist disputing parties identify important needs and interests, express grievances and develop mutually acceptable solutions in an informal and less adversarial atmosphere.  Funded by the Tennessee Administrative Office of the Courts.

Misdemeanor Probation Programs- The program provide supervision for those committing misdemeanor offences by monitoring court costs, fines, victim restitution, random drug testing, public service work and placement for drug/alcohol treatment as well as house arrest, continuing education services and values clarification treatment. Referrals are through General Sessions Court and are available to individuals who have committed felony offenses in Bledsoe, Grundy, Marion, Rhea and Sequatchie counties. Funded entirely by offender supervision fees.

Representative Payee . . . Top

SSA/SSI/VA Railroad Retirement- This program acts on behalf of recipients to manage finances on a monthly basis insuring that basic needs such as shelter, food and clothing, are met and provides a more stable lifestyle for the recipient. In order to be eligible for the program, the Social Security Office, Veterans Administration or Railroad Retirement must require an individual to have a Representative Payee. Available in Bedford, Bledsoe, Bradley, Franklin, Grundy, Hamilton, Marion, McMinn, Meigs, Polk, Rhea, Sequatchie and Warren counties and throughout the United States if the local Social Security Office and client agree. Funded entirely through participant fees.

Youth Programs . . . Top

Tennessee Prevention Network – The program services will address juvenile alcohol and drug prevention presented in an alternative school setting and a community based setting for after school programs.  Group sessions will be conducted four (4) days per week, one (1) hour per day. Referrals will be made to the program through the local alternative schools and Juvenile Courts. Services are available to youth ages 13-18 in Grundy, Marion, McMinn, and Rhea Counties

Juvenile Community Intervention - This program provides juvenile offenders an opportunity to repay the community for their crime and provides the court with an intensive juvenile probation services at no cost. Specialized counseling for juvenile and parents is provided, as well as random home visits, drug screens and community service. Juveniles who are referred by the court system in Franklin, Marion, McMinn, Meigs and Rhea Counties are eligible for services. The program is funded through the Tennessee Department of Children’s Services.

Transportation Services . . . Top

Public Transportation - Transportation is available to all residents of the nine county service area in Southeast Tennessee. The program allows almost unlimited access to facilities, shopping, doctors, etc through public transportation. Our goal in all transportation departments is to provide the most efficient, economical, safe transport for our clients as possible. The general public and contract clients are eligible to participate in public transportation. Funded by Tennessee Department of Transportation and fares.

Job Access Reverse Commute (JARC)- Provides clients transportation to destination such as job sites, training facilities.  The program also allows for transporting client’s children to and from day care facilities.  The program is available in Bledsoe, Bradley, Grundy, Hamilton, Marion, McMinn, Meigs, Polk, Rhea, and Sequatchie County.  Qualifying households must be within the guidelines set forth by the Tennessee Department of Transportation. Funded by Tennessee Department of Transportation.

Families First- The program Provides clients transportation to destination such as job sites, training facilities.  The program also allows for transporting client’s children to and from day care facilities.  Families First income guidelines are set forth by the Tennessee Department of Human Services and is only available in Bledsoe, Bradley, Grundy, Marion, McMinn, Meigs, Polk, Rhea and Sequatchie counties. Funded by Tennessee Department of Human Services.

Cleveland Urban Area Transit System (CUATS) - CUATS is a small urban transit system that serves in the urbanized area of Cleveland, Tennessee.  The system operates three fixed routes within the city, each with designated stops and transfer points.  A fourth route will be implemented in the near future.  CUATS is also equipped to pick up disabled clients at their homes. Hamilton County Division (STS) - Hamilton County Division provides contract services to local health and human services agencies.  The coordinated system reduces waste by using one fleet of vehicles to transport clients come to a number of social services agencies.  Passengers include disabled school children, Families First and Job access participants, dialysis and TennCare patients and local homeless network participants.  Eligibility is determined by the contracting agency. Funded by Tennessee Department of Transportation and contract agencies.
